Auto Claims Information

Fund members that are familiar with the P/L online claim forms will notice a few enhancements. We have included a link to give users the option of returning to the Fund's "Report a Claim" page and added a few questions that will require a response. Don't worry, the changes are minimal!

If you are a Fund member and are not reporting property or liability claims on line, we encourage you to do so. There are many benefits to online reporting so check out the forms now!

Immediately report accidents/incidents involving serious or fatal injuries to the Property and Liability Claims Division at 800.482.7276, extension 6800.

A hard copy of this claim form can be used internally in the district to document accidents. Refer to the "Downloads" box on the right side of this page for a printable copy.

Auto Liability

These claims involve bodily injury and/or property damage resulting from use or operation of a motor vehicle.  

Specified Perils

This type of limited coverage provides for losses such as: 

  • Fire or lightning
  • Theft or larceny
  • Windstorm, hail, earthquake, or explosion
  • The forced landing or falling of any aircraft or its parts or equipment
  • Malicious mischief or vandalism
  • Flood or rising waters

Comprehensive

These claims involve damage to your vehicle caused by something other than collision. The following are examples. 

  • Vandalism
  • Breakage of glass
  • Fire
  • Theft 
  • Hail 
  • Collision with an animal

Collision

This is a loss caused by collision of a covered auto with another object or with a vehicle.
